Abstract: Objective: The purpose of our work was to review through four observations the mechanisms of injury, the main signs calling for classification in the management of blunt external trauma to the larynx in a hospital of last resort in Mali. Observations: We reported a series of four cases of blunt external trauma to the larynx, all of which occurred in the context of a public road accident (AVP) with reception of the shock at the level of the anterior part of the neck. Dysphonia was the most constant sign, it was associated with dyspnea. All our patients had benefited from a Nasofibroscopy or a direct laryngoscopy and a pharyngolaryngeal computed tomography allowing them to be classified according to the classification of Schaefer and Fuhmann. A patient with arytenoid dislocation underwent endoscopic reduction. Exploratory cervicotomy was performed in one patient. Conclusion: Adequate management of external trauma to the larynx requires a structured, rapid and precise diagnostic and therapeutic approach. This constitutes a challenge in our context where the mobilization of the patient and the diagnostic means (nasofibroscopy or a direct laryngoscopy, pharyngolaryngeal computed tomography) in the absence of third-party payment are part of a course of overcoming obstacles.

Abstract: Congenital naevi are a benign proliferation of melanocytes. Present at birth or appearing in the first weeks of life, they have the appearance of light to dark brown lesions, sometimes bluish, more or less hairy. It is a non-hereditary mosaic pathology. They are due to postzygotic somatic mutations involving key proteins in the mitogen-activated protein kinase pathway, mainly NRAS and BRAF. Its prevalence is estimated to be between 1 and 3% of newborns. This disease can affect any part of the skin. There are 3 types of naevi: small, medium and large naevi then called giant. The objective was to discuss the clinical and therapeutic aspect of a congenital auricular naevus of medium size. We report the case of an 18 years old girl presenting a sessile tumefaction at the level of the concha of the right auricle that appeared from birth, firm, rough, unique, about 5 cm in diameter, homogeneous in color, pigmented, hairy, painless to palpation, apyretic sometimes pruritic without any other associated sign. The diagnosis of benign congenital naevus was clinically made after confirmation by the dermatologist. Surgical removal was performed. The postoperative course was simple with complete healing after one month. The middle congenital naevus is a benign tumor linked to a genetic variation of low incidence, which is all the more rare as it is located on the ear, thus having a significant psychological impact due to its often unsightly appearance. The diagnosis of this genodermatosis is essentially clinical. When it is an isolated nodule, as in our study, total surgical removal is a good alternative with simple after-effects and an almost non-existent risk of malignant transformation.

Abstract: The aim of this study was to establish the epidemiological, etiological and therapeutic mapping of adult deafness in a second referral hospital in Black Africa. Our retrospective study covered a period of one year from January 1 to December 31, 2018, in the ENT department of the CSREF of the commune I of the district of Bamako in Mali. The study focused on patients treated as outpatients for Deafness and presenting a complete file with a tonal audiometry form. During the study period, 39.28% of the patients admitted to outpatient clinics were adults, 10% had hearing loss, representing 3.9% of all admissions received. The male gender was the most represented, 54.67% with a sex ratio of 1.2. The 18-28 year olds were more numerous (40%), students/pupils 29.33%. Hearing loss was the most common reason for consultation with a rate of 53.33%. Tinnitus was the most frequent associated symptoms with a rate of 44%. The mode of onset was most often progressive (68%). The most common medical conditions were hypertension (22.66%), followed by meningitis (6.66%), with 53.33% of the cases having no particular medical condition. Trauma with 9.33% was the most mentioned risk factor. Chronic otitis media of all kinds was the dominant etiology 61%. 26.67% had a surgical indication and 33.33% a hearing aid. The prevalence of deafness in our context is relatively similar to the literature with similar epidemiological and clinical variables. Regarding the management, we note nevertheless some insufficiencies in particular as for the use of the tools of functional exploration in particular the vocal audiometry and especially at the level of the management but it should be noted that efforts are being accomplished concerning the levelling of the human resources, the surgery and the means of rehabilitation.

Abstract: Goal: The aim was to establish the epidemiological and clinical profile of patients diagnosed with pharyngo-laryngeal pathologies. Methods: This is a descriptive prospective study conducted in ENT heath care service over a period of one year. The present study was done in the ENT-HNS department of the Reference Health Center of Commune V of Bamako. We consulted and included all patients that were visiting for the first time for pharyngo-laryngeal pathology. In addition, former patients who came for a second or umpteenth consultation and those admitted to another department requesting an ENT opinion were not included. Results: The pharyngo-laryngeal pathologies represented 15.21% (658 patients) of the total heath examination done on 4326 patients. We identified 60% women and 40% men. The age group 0-10 years constituted 42.10% of the cases. The average age was 19 years, while the extremes were 12 days and 83 years. The reasons for consultation were as follow: odynophagia (47.42%), fever (42.71%), rhinorrhea (41.64%), nasal obstruction (25.84%), headache (18.84%), and a cough (12.92%). Angina accounted for 37.39%, nasopharyngitis 29.78%, pharyngitis 27.20%, phlegmon-peri-tonsillar 2.74%, adenoids and laryngitis each 0.76%, foreign bodies 0.61%, laryngomalacia 0.46%, hypopharyngeal tumors and laryngeal papillomatosis each accounted for 0.15% of diagnoses. Conclusion: Pharyngo-laryngeal pathologies are dominated by inflammatory and infectious pathologies.
