Abstract: As the SARS-COV-2 virus pandemic spreads around the world, new clinical manifestations are being reported. In addition to respiratory manifestations, acute kidney injury, hypercoagulability, pulmonary thromboembolism, and gastrointestinal symptoms have been reported. The Infectology Hospital of the La Raza National Medical Center, in the Intensive Care Unit, has treated 148 patients from November 2020 to November 2021 for severe to critical COVID-19. We present the case of a patient with SARS-COV-2, with intestinal perforation due to vascular congestion at the level of the cecum in a critical COVID-19 patient.

Abstract: Background: Sarcoidosis can affect virtually any organ of the body, primarily the lungs, lymphatic system, skin or eyes, or a combination of these sites and it is characterized by the formation of non-caseating granulomas. Muscular involvement in sarcoidosis is usually asymptomatic or pauci-symptomatic, and its nodular form is exceptional. Observation: In this study, we report the case of a 36-year-old man who had been presenting with nodules in both calves for about two years which onset was progressive. These nodules were painless and deeply embedded in the gastrocnemius muscles. There were no functional complaints, and the general condition was preserved. The physical examination, especially at skin level was free of other clinical signs except the nodules. The PET-scan (positron emission tomography) revealed a few discretely hypermetabolic cervico-axillary lymph nodes and multiple bilateral mediastino-hilar adenopathies involving all lymph nodes, as well as multiple bilateral hypermetabolic pulmonary foci. Biopsy of accessory salivary glands (BASG) exhibited an epithelioid and gigantocellular granuloma without necrosis that confirm the diagnosis of systemic sarcoidosis. The search for another location, notably cardiac, ophthalmological, neurological, or renal, was negative. The respiratory function tests did not show any restrictive syndrome or abnormalities in the DLCO/VA ratio. It was therefore a systemic sarcoidosis with mediastino-pulmonary stage II, diffuse lymph node, splenic, and muscle involvement. Conclusion: This case shows the importance of BASG in the diagnosis of muscular sarcoidosis, in case of impossibility of performing a muscle biopsy.

Abstract: Chronic renal failure is a public health problem worldwide because of its high mortality rate and the cost of its management. Our study aims to determine the mortality factors related to CKD at the University Hospital of the National Reference of N’Djamena (Chad). This was a cross-sectional, descriptive study over a period of three (3) years from January 1, 2016 to December 31, 2018. The study focused on the records of patients hospitalized and whose death was related to chronic renal failure at the CHU-RN of N'Djamena. The mortality rate related to CKD was 10.52%. The mean age was 48 ± 16.3 years with a sex ratio of 1.2. The main causative nephropathies were hypertension and diabetes. 83,8 % were in the terminal stage of CKD. Hemodialysis was performed in 17 patients (21.3%). The average length of hospitalization of our patients was 20.1± 16.9 days. Hemodynamic PAO was the main cause of death. One third of the deaths occurred between 00:00 and 5:59 with a frequency of 30%. Deaths occurring between 12:00 and 17:59 minutes represented 21%. In developing countries, CKD is diagnosed in the end stage as indicated by our study (83%). Hemodialysis remains the best treatment to reduce the mortality rate.

Abstract: Background: Chronic hepatitis C virus (HCV) infection induces the occurence of severe chronic liver disease and metabolic complications. Our goal was to study the clinical and paraclinical profile of patients with hepatitis C virus at the Natioanl Teaching Hospital Hubert Koutoukou Maga of Cotonou. Methods: This was a cross-sectional and descriptive study conducted at the Teaching Clinic of Hepato-Gastroenterology of the Natioanl Teaching Hospital Hubert Koutoukou Maga of Cotonou with the treated patients, from January 1, 2012, to October 31, 2018. Results: A total of 80 patients with hepatitis C were included in the study. The mean age was 60.2 ± 10.8 years old with extremes of 34 and 84 years. Most of the subjects surveyed were women (62.5%) with a sex ratio of 0.6. Arterial hypertension was the prevalent history in these patients (61.2%). HCV carriage was known for less than 5 years in 70% of cases. The most represented genotype was viral genotype 2 (57.2%). Complications were noted in 47.5% of respondents, dominated by cirrhosis (40%). FibroTest was the main method of non-invasive evaluation of fibrosis. Conclusion: In our study, hepatitis C affects mostly women. It is often diagnosed late. Hence, there is a real need to reinforce awareness.

Abstract: Background: Sudden cardiac death (SCD) is the leading cause of mortality in athletes during sport. There are several hereditary, structural, or electrical heart diseases associated with SCD, most of them can be identified or suggested by abnormalities on a resting 12-lead electrocardiogram (ECG). Objective: To evaluate usefulness and cost-effectiveness of ECG screening, using a telecardiology system, in order to detect cardiovascular diseases that might cause SCD in a cohort of non-competitive athletes (NCA). Methods: ECGs received at the Telecardiology Center (Telemedico Srl, Genoa) for non-competitive sports, in a 3-month period were analyzed. According to the age we distinguished: NCA < 35 years (group A) and NCA > 35 years (group B). Results: A total of 6345 NCA (4078 males, 64.2%) were evaluated between the ages of 5 and 50 years (mean ± SD: 28.4 ± 12.6). Group A (age < 35 years) consisted of 3453 (54.4%) NCA and group B (age > 35 years) of 2892 (45.6%) respectively. An ECG pattern at risk of SCD has been identified in 295 (4.6%) subjects. Prevalence of ECG abnormalities was higher for group B (154/2892; 5.3%) than group A (141/3453; 4.1%); the difference was significant (p= 0.016). The average cost per ECG was € 12. The cost of ECG screening to identify a risk situation of SCD was € 258. Conclusion: ECG screening through telecardiology of NCA results to be feasible, useful and cost-effective.
