Trends of Spontaneous Intracerebral Hemorrhage in Florence District Along Fifteen Years: A Brief Report

Abstract: Spontaneous intracerebral hemorrhage (ICH) represents the most feared stroke subtype. Real world epidemiological data about trends in incident cases and in-hospital mortality lack. Therefore we performed this study aimed to answer this concern. International Classification of Diseases, 9th revision, Clinical Modification (ICD-9th CM) database referred to patients discharged from six Hospitals of Florence district, Tuscany, Italy in a period fifteen years long (2001-2015)was analyzed. We searched for code 431 as primary or secondary diagnosis at hospital discharge. Overall, 7452 patients were discharged with ICH as primary or secondary diagnosis. Of them, 3695 (49.5%) were females and 4363 (59.1%) were 75-years old and over. Cases of ICH increased from 461 in 2001 to 568 in 2015. The greatest increase was observed in patients 75 years old and over (216 cases in 2001, 339 cases in 2015). Overall, 2273 patients died during hospital stay, in-hospital mortality being 30.5%. In-hospital mortality increased according to age, being 18% in under 65 years and 35.9% in 75-years old and over. In-hospital mortality decreased from 30.8% in 2001 to 25.1% in 2015. The decrease in in-hospital mortality was observed irrespective of age. In Florence district, cases of ICH increased over the years, especially in very old people, whereas in-hospital mortality decreased irrespective of age.

Abstract: The auto immune myasthenia comorbidity and Biermer disease is less documented and rarely brought in the literature. We bring back the observation of a Senegalese patient hospitalized in our department of Neurology at Fann hospital (Dakar). It concerned a patient aged 58 years followed up for auto-immune myasthenia to antibodies anti-receptors of acetylcholine Ac RACH since 8 years and having a benefit of recurrent blood transfusion with a blood group A rhesus positive. He was received on neurologic consultation for a tiredness associated to an effort dyspnea and a gastro esophageal reflux accompanied by vomiting. The interrogatory found palpitations which necessitated a hospitalization two month before. Physical examination had objective a myasthenia syndrome, an anemic syndrome on the other hand, sub icteric mucosa’s were noted but no melanodermia no glossite. The rest of the physical examination was without particularity. The diagnoses of the Biermer illness was carried out in front of the anemic syndrome, the chronicity of the symptomatology without notion of fluctuation and the complementary exams having as objective a low rate of hemoglobin and a deficit in vitamin B12. The origin auto-immune of this anemia was confirmed by the immunologic test which had put in evidence a high rate of anti-bodies anti- intrinsic factor. The patient benefited from a blood transfusion then a treatment from cobalamin (for life) was also installed associated to a symptomatic management of the patient. The evolution after 6 weeks of the treatment was favorable with a complete regression of the dyspnea, vomiting and the attenuation of the effort tiredness. Myasthenia gravis and Biermer disease comorbidity has to be discussed in front of every myasthenia patient presenting clinical signs of effort dyspnea to a chronic anemia because early diagnoses of this association of Biermer illness and myasthenia gravis favors a better prognosis and not to progress to the combined degeneration of the spinal cord. The autoimmune substratum of the mechanism of this comorbidity remains to be elucidated but in all cases multidisciplinary management is necessary.

Abstract: A Quantitative Histopathology study on rats’ brainstem was used to analyze morphological alterations in the neurons and glial cells of rats that received inhaled tetrahydrocanabinol for 4, 8 and 12 weeks. Puffing of smoke was performed with the use of a Hamilton syringe delivering 100ml puffs at 20s intervals into the nose only manifold. Smoke was first pumped into a 500ml dilution chamber with the aid of a vacuum pump. The smoke was then displaced from the dilution chamber through the nose-only manifold at 300ml/min; the rats received inhaled THC at 5ml/sec for 5 minutes. After administration for varying durations a selective cell staining of the neurons and glial cells in the rat’s pons, medulla and midbrain was carried out and used to study visible morphological changes in the tissues. Sections from the pons, medulla and midbrain were stained on slides for viewing under the microscope and photographed. Quantitative and qualitative histopathology study of photomicrographs was then used to analyze changes in the morphology and number of neurons. There was an increase in the ratio of neuronal cells comparing between the control and the treated groups with the pons (1:8), medulla (1:3) and the midbrain (1:5) which suggests neurogenesis and on further analysis of the slides show evidence of cell division. These findings can be of great importance in the study of neurodegenerative diseases and in understanding the influence of THC on brain.

Abstract: In many world, research of centella asiatica (CA) leaf extracts are used to treat neuronal disease with stem cell or cell line, but in the present study we try to make from hippocampus tissue culture. Methods: Young femalemice dowleyspraque types age of 2 weeks, as many as eighteen (18) tails each taken from its hippocampal tissue then cultured using a medium Roswell Park Memorial Institute (RPMI). Eighteen (18) culture of nerve cells exposed to the extract of CA at a concentration of 0.25 ug / ml (CA A) and eighteen (18) culture of nerve cells exposed to the extract of CA at a concentration of 0.50 ug / ml (CA B), where as eighteen (18) other nerve cell cultures not exposed (control). Then calculated the number of cells per field view and BDNF levels measured in the culture of the three populations. Results: (1) Anumber of larger nerve cells in cell cultures of hippocampal tissue of young rats were fed by CA extracts dose of 0.50 ug / ml compared to 0.25 ug / ml and the control group (p <0.05); that new neurons plays an important role in sinaptogenesis circuit in order to improve the integration function of neurons and synapses that still exist between new and mature neurons in the hippocampus [1]. Conclusion: (2) Levels of BDNF CA dose of 0.50 ug / ml higher than 0.25 mg/ml CA doses but lower than in the controls (p <0.05), that the effect of the extract CA neurogenesis does occur in particular BDNF through the expression of BDNF, the possibility of glutamate in CA extracts is not able to increase the level of GABA in the brain due to activation of GABA synapses on a young neuron is depolarization where the concentration of intracellular Cl ion is high because channel expression Cl exporter KCC2 which slowed (delayed) [2].

Abstract: We report a patient who showed mixed cytotoxic and vasogenic edema on Magnetic Resonance Imaging (MRI) in association with partial status epilepticus. The patient experienced clonic movements of the left arm and leg. As the clonic movements continued, resulting eventually in epilepsia partialis continua, MRI showed characteristic cytotoxic edema in the epileptogenic focus and vasogenic edema in the region remote from the epileptogenic focus. The aim of this report is to support the evidence that partial seizure spreads through efferent projections from the cerebral cortex to the cerebellum, and findings of serially followed MRI suggest that cytotoxic edema persisted for a long time in contrast to vasogenic edema.
