Abstract: It is imperative that healthcare systems, regardless of their source of funding, are sustainable, provide quality care and develop towards their customers’ expectations. This study aims at a comparative analysis between two hospital centres, one belonging to the Portuguese National Health Service and the other to the British National Health Service, in order to identify which strategies have been adopted by their hospital management to reduce health costs without prejudice to the quality of the services provided. To this end, semi-structure interviews were conducted with the financial directors of both hospitals. The questions presented focus on changes in terms of financing, the relationship between costs and quality, improving efficiency by cutting costs, strategies for reducing waste and operating vectors for improving efficiency. In the Portuguese case, his perspective focuses on the management and organisation of services from a perspective intrinsic to the organisation, characterised by restructuring and innovation, based on close and cooperative interrelations between sectors and hierarchies. In the British testimony, we verify a perspective more focused on the outside of the organisation, placing its focus on an inter-relational triangulation between population, organisations and professionals. The focus on health professionals, technology, reorganisation of services and waste stands out as being four fundamental pillars of action for an efficient cost reduction, without impairing quality, considered as areas of intervention relevant to the development of future studies. We also highlight the managers’ concern in cost reduction without prejudice to quality and in promoting services adjusted to the needs of the users.

Abstract: The main focus of this review is on Ethiopia's rural households' current food insecurity, including its causes, effects, and coping mechanisms. At the international, regional, national, and local levels, there is a direct correlation between food insecurity and poverty. According to the most recent studies, 842 million people or 12 percent of the world's population were unable to meet their dietary energy needs globally. According to recent studies, Ethiopia is one of the world's poorest nations, with a significant portion of its population living below the poverty line and the majority of the rural population (roughly 9.7 million people) experiencing food insecurity. Coping Strategies against Food Insecurity used by farm households in the rural area of Ethiopia include livestock sales, agricultural employment and, sale of wood or charcoal, small scale trading, selling cow dung and crop residues, reduction of food consumption. Finally, the current food security policy programs in Ethiopia are Sustainable Production & Market Systems, Business & Entrepreneurship, Resilience Nutrition, Cross-Cutting Issues, Small scale irrigation and Policy options to minimize food insecurity based on Strengthen productivity and incomes, Linkages maximizing synergy and Provide direct access to food. Improve the coping mechanism of the rural households by improving their capacity to protect themselves from food insecurity through enhancing their knowledge on preservation and storage of food and credit schemes, diversification of income, and employment.

Abstract: The Tanzanian government, through the Ministry of Health, in collaboration with President’s Office, Regional Administration and Local Government (PORALG) and various stakeholders, has developed several robust electronic health systems such as e-LMIS, GoTHOMIS, FFARS and DHIS2 to streamline health supply chain management processes and improve data visibility. However, the use of this data for informed decision making at all levels of the supply chain was found to be very low, which led to the introduction of IMPACT (Information Mobilized for Performance Analysis and Continuous Transformation) to increase the use of data to improve the supply chain in the country, including Sikonge District Council. Following the implementation of the approach, the IMPACT team in Sikonge discovered low performance in the availability of health commodities among other monitored indicators major reason being inadequate financial resources to procure health commodities. This prompted the team to look for other innovative ways of fundraising to rescue the situation in the council. The objective of this study was therefore to assess the influence of the IMPACT approach methods employed in mobilizing financial resources to improve the availability of health commodities in Sikonge DC. Some of the methods employed by Sikonge team for such a purpose were; conducting a preliminary data-driven analysis and assessment to identify issues related to i-CHF and NHIF services, developing strategies to increase i-CHF enrolment, retention, and disbursements, working on the most common anomalies in the NHIF forms to reduce the number of rejected claims and finally clearing the MSD debt of health facilities to relieve them of the financial burden. These strategies resulted in an increase in i-CHF funds reimbursement from Tsh1.4 million in September 2020 to Tsh8 million in December 2021. NHIF funds reimbursement increased from Tsh696,170 in September 2020 to Tsh12 million in December 2021. The overall percentage of health facilities submitting NHIF claims increased from 17% to 77% over the same period. Thus, the concept of IMPACT and the employed data driven strategies, played a major role in the results achieved in Sikonge and in order to meet the alarming demand for health commodities at service delivery points, council teams should show strong commitment in using real-time data for problem identification and resolution through IMPACT team approach as a tool for continuous improvement.

Abstract: Dental avulsion is one of the most common acts performed in dentistry. It concerns both the young population, but its prevalence increases with age. The objective of this work was to study the socio-economic determinants and the demand for dental avulsions at the Institute of Odontology of the Cheikh Anta Diop University of Dakar (UCAD). This was a descriptive cross-sectional study of patients who were seen in the oral surgery department of the UCAD Odontology Clinic. Data were collected from medical records from October 14, 2019 to March 28, 2020. Tools used to collect and process the information were Sphinx software and Excel spreadsheet. From this, it was found that of the 525 patient records, 60% were male and 52.19% were between the ages of 20 and 39 years. More than half (53.14%) of patients lived less than 5 km from the health care facility and 42.7% of patients had a monthly income of between 50,000 and 100,000 FCFA. Clinically, 49.9% of patients came for an odontalgic reason and periodontitis followed by coronal destruction were the most diagnosed pathologies. Also, there is a relation between diagnosis and the variables age and income with respectively P-value 0.023 and 0.001. Closeness to health facilities, income, and cost of treatment are factors that influence accessibility to oral health care. It is important to integrate the health care consumer's profile in the overall approach to care.
